Insurance Information:

Please note that TPMD is considered an “Out of Network Provider” and does not participate in any insurance companies. Please inform our team if you have medicare or medicaid benefits prior to any evaluation. At the end of each visit you will be provided with a receipt and “superbill” which can then be submitted to your insurance company for reimbursement if you have out of network benefits. Patient/family is responsible for obtaining information and knowledge about their health insurance benefits.
